• Contract - 1 original notarized
• Contract Addendum - 1 original
  
• Medical Release Form - 1 original notarized
• Medication Permission Form - 1 original & 2 copies
• Travel Rules - 1 original
• Credit Card Authorization Form - for credit card pymts
• Parent Handbook

We will have someone on staff to notarize all documents at time of signing. 

Contract and Contract Adddendum forms will not be posted online as they will be given to the player when they are offered a position on one of our teams.

ITEMS NEEDED AT TRYOUTS

2009-2010

1. Rocky Mountain Region Registration (New Member) (Renewing Member) - 1 Copy of your PAID ($45) Confirmation Form. Junior Membership-RMR Powers, Regional, & National Events 
2. 3 Copies of Birth Certificate, Passport or Drivers License
3. 1 Original & 2 Copies - USAV Medical Form  
4. 1 Original & 2 Copies - COVOCO Player Information Sheet
5. $50 tryout fee (cash or check only)
6. $850 Fee Deposit (deposit of $850 is required at tryouts.  If a position is not offered and/or accepted the check or credit card information will be destroyed.  Upon acceptance the $850 will be deposited or processed.

You must have everything listed above before you can tryout with COVOCO.
